We have partnered with Navigating Care's patient portal to offer a secure online website that gives you, as a patient convenient 24-hour access to your personal health information and medical records from anywhere. You may also designate a personal representative to access to this information electronically on your behalf.
This portal offers you secure, instant access to your Personal Health Record (PHR), which includes information about your diagnosis, medication list, lab results, and clinical documents. When registered for the Navigating Care portal, you can also securely message your care team directly, see your upcoming appointments and report symptoms directly from the home page of the portal.
